Consent to the processing of personal data

CONSENT

pursuant to Article 6(1)(a) and Article 7 of Regulation (EU) No 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) and pursuant to Article 13(1)(a) and Article 14 of Act No 18/2018 Coll. on the protection of personal data and on amending and supplementing certain acts

As the data subject, I hereby give my voluntary and explicit consent to the processing of my personal data in the scope of the data filled in the items name and surname, telephone contact, e-mail, if I have provided them.

Processing of the proposal for modification of the classification system, which is defined in particular in Section 67a(5) of Act No. 581/2004 Coll . on Health Insurance Companies, Supervision of Health Care and on Amendments and Additions to Certain Acts. The processing of complaints is carried out by employees of the Ministry of Health of the Slovak Republic. Contact details are collected for the purpose of obtaining additional information on the proposals, if necessary, and informing the submitter of the proposal about the resolution of the proposal for the period until the withdrawal of the imposed consent.

The explicit consent may be revoked at any time by sending a request to the operator's e-mail address office@health.gov.sk or to the address of the operator's registered office, Ministry of Health of the Slovak Republic, Limbová 2, P.O.BOX 52, 837 52 Bratislava.

As a data subject, I declare that I am aware of my rights under Act No. 18/2018 Coll. on the Protection of Personal Data and on Amendments and Additions to Certain Acts.

The controller shall ensure that the personal data of the data subject are sufficiently protected against theft, against accidental or targeted access by unauthorised persons, and against any other impermissible forms of processing. It shall create conditions such that the confidentiality and integrity of personal data are not compromised in the processing of personal data.

The controller will not transfer the data subject's personal data to third countries or to an international organisation.

The controller shall process personal data for the period necessary to fulfil the purpose.

7.1 the right to require the controller to have access to the personal data of the data subject (§ 21 of the Act),

7.2 the right to erasure of personal data (§ 23 of the Act),

7.3 the right to rectification of personal data (§ 22 of the Act),

7.3 the right to restrict the processing of personal data (§ 24 of the Act),

7.4 the right to object to the processing of personal data (§ 27 of the Act),

7.5 the right to portability of personal data (§ 26 of the Act),

7.6 the right to withdraw their consent at any time (§ 14 (3)),

7.7 the right to institute proceedings under (section 100 of the Act).

The controller shall not process the personal data of the data subject by automated individual decision-making, including profiling.
